When should you consider an andrology consultation?

An andrologist diagnoses and treats health problems related to the male reproductive and hormonal systems. An appointment may be indicated if one or more of the following symptoms occur:

  • Reduced libido — decreased interest in sexual activity lasting for a longer period,
  • erectile dysfunction — difficulty achieving or maintaining an erection,
  • fertility problems — difficulty conceiving a child or abnormal semen analysis results,
  • chronic fatigue and weakness without a clear cause,
  • symptoms that may suggest testosterone deficiency — loss of muscle mass, increased body fat, low mood, concentration problems or hot flushes,
  • pain or changes in the testicles requiring specialist assessment,
  • a desire to assess the hormonal profile — preventively or following a referral from another doctor.

Symptoms may have different causes and are not always due to testosterone deficiency or hormonal disorders. Medical assessment helps identify the source and plan appropriate management.

What happens during an andrology consultation?

The appointment is diagnostic and individual. The doctor conducts it in stages:

Detailed medical history
Discussion of reported symptoms, how long they have been present, general health, medications, chronic diseases, lifestyle and the patient’s expectations.

Physical examination
Assessment of body build, genital organs, testicles and epididymides. If indicated, the prostate may also be examined.

Additional diagnostics, if indicated
The doctor may order hormonal tests, including total and free testosterone, LH, FSH, prolactin, estradiol, SHBG and PSA, testicular ultrasound with blood-flow assessment, semen analysis or other tests depending on the clinical picture.

Discussion of results and management plan
After collecting all relevant information, the doctor discusses the diagnosis or diagnostic hypotheses and presents possible treatment options or further investigations.

What can an andrologist offer?

Management depends on the diagnosis and test results. Possible treatment approaches include:

  • Testosterone replacement therapy (TRT) — used in confirmed hypogonadism after contraindications have been excluded. It requires regular monitoring of blood parameters, PSA and haematocrit. The decision to start therapy belongs solely to the doctor.
  • Pharmacological treatment of erectile dysfunction — PDE-5 inhibitors or other medications selected individually and with consideration of coexisting conditions.
  • Management of fertility disorders — depending on the cause: medication, lifestyle changes or referral to a fertility specialist.
  • Lifestyle modification — recommendations regarding diet, physical activity, sleep and supplementation as supportive measures, not a replacement for treatment.
  • Referral to another specialist — if the problem requires endocrinological, psychiatric, sexological or urological-surgical care.

Important: Hormonal therapy (TRT) and medications for erectile dysfunction have indications, contraindications and potential side effects. They are not suitable for every patient. Treatment decisions require medical assessment, analysis of test results and — in the case of TRT — regular monitoring. No therapy should be started without medical supervision.

What should you bring to an andrology consultation?

The appointment may proceed more efficiently if the patient brings available documents: previous hormonal or blood test results, testicular or prostate ultrasound results, semen analysis if previously performed, a list of medications and supplements, and information about chronic diseases and previous operations. This is not mandatory — the patient may attend without documentation, and the doctor can begin the diagnostic process from the start.

What is the difference between an andrologist and a urologist?

A urologist deals with the entire urinary and genitourinary system in men and women, including the prostate, kidneys, bladder and urethra. An andrologist specialises in men’s health problems, including hormonal disorders, sexual function and fertility. In practice, the scope of both specialties often overlaps, and some doctors combine both. The choice of specialist is best based on the symptoms reported.

Which hormonal tests are performed during andrology diagnostics?

The most commonly ordered tests include total and free testosterone, LH, FSH, prolactin, estradiol, SHBG and PSA. The scope of testing is selected individually by the doctor based on the medical history and clinical picture — not every patient requires a full panel.

Does a low testosterone level always require hormonal treatment?

No. The decision to start testosterone therapy (TRT) depends not only on test results, but also on the clinical picture, symptoms, coexisting conditions and contraindications. TRT has indications, limitations and potential side effects and is not suitable for every patient with low testosterone. Diagnosis and treatment decisions belong to the doctor.

Is an andrology consultation discreet?

Do I need a referral for an andrology consultation?

No. The appointment is private and no referral is required. You may bring existing test results or undergo diagnostics arranged after the consultation.

When does erectile dysfunction require urgent medical attention?

Sudden erectile dysfunction in men after a heart attack or stroke, or when accompanied by chest pain, requires urgent cardiological assessment rather than an andrology consultation. Erectile dysfunction may be an early sign of cardiovascular disease. If it develops suddenly in a young man or is accompanied by other vascular symptoms, tell the doctor during the appointment.
