Glans enlargement with hyaluronic acid
Glans penis enlargement with platelet-rich fibrin (PRF) is an outpatient procedure involving the injection of a preparation made from the patient’s own blood into the glans. PRF is not a conventional synthetic filler. This autologous preparation may influence tissue biologically, while the extent and duration of the result vary between patients. What is platelet-rich fibrin (PRF), and how does it work?
Platelet-rich fibrin (PRF) is a preparation obtained from the patient’s own blood. After collection, the blood is centrifuged according to a specific protocol to obtain a preparation containing fibrin, platelets and biological components involved in tissue repair processes.
Typical PRF protocols do not use an anticoagulant. The resulting fibrin network forms a natural matrix from which biologically active components may be released gradually. The precise composition of the preparation depends on the system used, centrifugation parameters and the individual properties of the patient’s blood.
PRF compared with other methods — how is this procedure different?
PRF is not a conventional filler such as cross-linked hyaluronic acid. It does not make it possible to plan or predict a specific increase in volume in the same way. Immediately after the procedure, the visible change may partly result from the volume of the injected preparation and temporary swelling. Once these subside, the result may be more subtle.

Who may consider a PRF procedure?
The procedure may be considered by adult men who expect a subtle change in the appearance or volume of the glans and accept that the result is not fully predictable. The doctor decides whether the procedure can be performed during a urology consultation. Qualification includes, among other things:
- assessment of general health,
- discussion of the patient’s expectations and the limitations of the method,
- assessment of penile anatomy and tissue condition,
- exclusion of infection and other contraindications,
- discussion of alternative methods,
- presentation of possible adverse effects.
Not every patient will qualify for this procedure.
How is the procedure performed?
The doctor takes a medical history and assesses the local condition and the patient’s general health. The expected result, limitations of the method, possible complications and alternatives are discussed. On this basis, the doctor decides whether the patient qualifies.
An appropriate amount of the patient’s venous blood is collected during the appointment.
The collected blood is centrifuged according to the protocol appropriate for the system being used. This produces the preparation intended for injection.
Before the injections, local anaesthesia selected by the doctor is administered, taking into account the extent of the procedure and the patient’s individual needs.
The doctor injects PRF into selected areas. The technique, number of injection sites and amount of preparation are determined individually.
The doctor assesses the treatment area and provides instructions concerning hygiene, physical activity, sexual activity and the follow-up appointment.
Recovery and post-procedure recommendations
Immediately after PRF administration, swelling, tenderness or pain, redness, minor bruising, injection marks, temporary irregularities or asymmetry, increased sensitivity or a temporary change in sensation may occur. These symptoms usually subside gradually, although their severity and duration may vary.
A return to light daily activities is usually possible the following day. The final decision depends on the patient’s condition and the doctor’s recommendations.
- Avoid sexual activity for approximately 5–7 days.
- Limit strenuous physical activity for approximately 7–10 days.
- Avoid hot baths, saunas, hot tubs and swimming pools until the injection sites have healed.
- Maintain appropriate hygiene of the treatment area.
- Use only medicines and preparations approved by the doctor.
- Attend the follow-up appointment at the scheduled time.
Individual instructions provided by the doctor take precedence over general information.
When should you contact a doctor urgently?
Urgent contact with the clinic is required in the event of increasing or severe pain, rapidly worsening swelling, extensive bruising or a haematoma, marked paling or darkening of the tissues, fever or signs of infection, pus-like discharge, difficulty urinating, persistent sensory disturbances or any symptom that causes concern. Possible adverse effects and complications
Although the preparation is made from the patient’s own blood, the procedure is not risk-free. Possible adverse effects include, among others:
- pain and swelling,
- bruising or a haematoma,
- bleeding from an injection site,
- infection,
- temporary or persistent sensory disturbances,
- irregularities and asymmetry,
- an unsatisfactory or imperceptible aesthetic result,
- the need for further treatment,
- damage to a blood vessel, nerve or other anatomical structures.

Contraindications
Possible contraindications include:
- an active skin or genital infection,
- inflammation at the intended injection site,
- blood-clotting disorders,
- thrombocytopenia and certain disorders of the haematopoietic system,
- severe anaemia,
- active cancer,
- certain autoimmune diseases,
- uncontrolled diabetes,
- taking medicines that affect blood clotting,
- inability to follow post-procedure instructions,
- unrealistic expectations regarding the result.
Important: Taking anticoagulant or antiplatelet medication does not automatically mean that it can be discontinued. Patients must not stop treatment on their own. Does PRF actually enlarge the glans?
A change in tissue volume and appearance may occur after the preparation is administered, but its extent is individual and difficult to predict. PRF is not a conventional filler, so a specific increase in circumference or permanent enlargement of the glans cannot be guaranteed. Immediately after the procedure, part of the visible effect may result from swelling. Patients should be aware that no noticeable aesthetic effect is one of the possible outcomes.
How does PRF differ from hyaluronic acid?
Hyaluronic acid is a synthetic filling material administered from outside the body and generally allows a more direct effect on tissue volume. PRF is an autologous preparation and does not act like a conventional filler. The procedures differ in their mechanisms of action, predictability, duration and risk profiles. The choice of method should be preceded by an assessment of the patient’s anatomy, health and expectations.
How does PRF differ from PRP?
PRP, or platelet-rich plasma, is usually liquid and may be prepared using an anticoagulant. PRF, in contrast, forms a fibrin matrix containing platelets and other cellular components. This does not mean that one method is always better than the other. The properties of the preparation depend on the protocol used, and the choice should be based on the objective of the procedure and the doctor’s assessment.
How long does the result last?
The exact duration of the result cannot be determined in advance. It depends on the type of preparation, injection technique, the individual tissue response and the expected outcome. In some patients, the change may be short-lived or minor. An unsatisfactory aesthetic result is also possible.
Is the procedure painful?
The procedure is performed under local anaesthesia. During anaesthesia and injection, the patient may feel needle pricks, pressure or temporary discomfort. Mild to moderate tenderness, swelling or pain may persist after the procedure.
How many sessions are required?
The doctor assesses whether the procedure should be repeated after the tissues have healed and the result of the first treatment has been evaluated. A further session should not be planned solely on the basis of temporary swelling immediately after administration.
When can sexual activity be resumed?
Sexual abstinence is usually recommended for approximately 5–7 days. This period may be extended if swelling, pain, bruising or incomplete healing of the injection sites persists. The final decision should follow the doctor’s recommendations.
What risks are associated with administering PRF to the glans?
Using autologous material reduces the risk of a reaction to a foreign substance, but it does not eliminate the risks associated with the injection itself. Safety depends, among other factors, on appropriate qualification, the conditions in which the procedure is performed, knowledge of anatomy, injection technique and compliance with post-procedure instructions. Are laboratory tests required before the procedure?
The doctor determines the scope of testing during the consultation. Depending on the patient’s health, medication and medical history, the doctor may request tests including a complete blood count and coagulation studies. Normal test results do not replace a medical consultation and examination.
