What is suspensory ligament release?
Suspensory ligament release, also called penile ligamentotomy, is a urological procedure that involves cutting the suspensory ligament — the structure connecting the penile shaft with the pubic bone. This ligament holds part of the penis inside the body, which means that in the flaccid state this part remains invisible. After the ligament is cut, the hidden part moves outward, increasing the visible penile length in the flaccid state.
The procedure does not interfere with the blood vessels or nerves responsible for erection — sexual function is preserved. It is worth knowing that the erection angle may change, which the doctor discusses with the patient in detail before a decision is made.
Who is the procedure intended for?
- men experiencing psychological or aesthetic discomfort related to penile length in the flaccid state,
- patients with anatomically buried penis, including in the course of obesity,
- patients who want to improve proportions and confidence in intimate life,
- men considering combining the procedure with hyaluronic acid penile modelling.
What does the procedure involve?
- Consultation and qualification. The doctor carefully assesses indications, discusses the course of the procedure, recovery and realistic expectations regarding results.
- Preparation and anaesthesia. The procedure is performed under local anaesthesia or sedation — the type of anaesthesia is determined individually by the urologist.
- Cutting the suspensory ligament. This is done through a small surgical access in the pubic area. The procedure usually takes 60–120 minutes.
- Wound care and recommendations. The patient returns home the same day with written recovery instructions.
- Postoperative rehabilitation. A key element is the systematic use of penile weights for several hours a day for several months — this prevents the tissues from returning to their previous position and helps maintain the effect.
Recovery after the procedure
- swelling and tenderness in the pubic area may persist for a few days and gradually subside,
- return to office work and light activity usually after a few days,
- bath, sauna and swimming pool should be avoided for at least 2 weeks,
- sexual activity should be avoided for 4–6 weeks, according to the doctor’s instructions,
- from week 2, according to recommendations, systematic use of penile weights for several hours a day for 3–6 months,
- follow-up visits at scheduled times — the doctor monitors healing and adjusts recommendations,
- full results are visible after 6–12 months, once the tissues have fully stabilised.
Contraindications
- active infections or inflammation in the genital area,
- blood clotting disorders or use of blood-thinning medication,
- uncontrolled chronic diseases such as diabetes, heart disease or immune disorders,
- lack of readiness to follow the rehabilitation protocol with weights,
- unrealistic expectations regarding the effects of the procedure,
- age under 18.

By how many centimetres does the penis lengthen after the procedure?

Does the procedure hurt?
The procedure is performed under local anaesthesia or sedation, so the patient does not feel pain during it. Tenderness and swelling may persist for a few days after the procedure and gradually subside. The doctor prescribes appropriate pain relief.
Does erection change after the procedure?
Erectile function is preserved — cutting the ligament does not affect the vessels or nerves responsible for erection. However, the erection angle may change: during erection, the penis may point slightly more downward. This is important information that the doctor discusses in detail before the decision is made.
Why are weights used after the procedure?
After the ligament is cut, tissues tend to return to their previous position, which is called retraction. Systematic use of penile weights for several hours a day for several months helps prevent this process and maintains the lengthening effect. Can the procedure be combined with hyaluronic acid enlargement?
Yes. Ligamentotomy provides a lengthening effect, while hyaluronic acid modelling increases penile circumference. The possibility and timing of combining both procedures are decided by the doctor based on an individual patient assessment.
When can I return to sexual activity?
Sexual activity should be avoided for 4–6 weeks to allow proper tissue healing. The exact time is determined individually by the doctor and verified during follow-up visits.
Are the results permanent?
With proper postoperative rehabilitation using weights, the results are long-term. Following the doctor’s recommendations in the first months after the procedure is crucial and largely determines the final outcome.
