What is erectile dysfunction?

Erectile dysfunction, formerly called impotence, is difficulty achieving or maintaining an erection sufficient for satisfying sexual intercourse, lasting for at least several weeks. It is one of the most common urological problems in men — it is estimated to affect 30–40% of men over the age of 40 to varying degrees, and more than half of men over 60.

Erectile dysfunction is not a reason for shame and not a life sentence. In most cases, it has an identifiable cause — vascular, hormonal, neurological, medication-related or psychological — and may be treated or significantly improved with proper diagnostics.

Symptoms — how to recognise erectile dysfunction?

  • difficulty achieving an erection, even with sexual arousal,
  • an erection appears but is not sufficiently firm,
  • the erection disappears quickly before or during intercourse,
  • reduced frequency or intensity of night-time and morning erections,
  • decreased libido accompanying erection problems, which may suggest a hormonal background.

If these symptoms recur for several weeks and affect quality of life, it is worth discussing them with a urologist or andrologist — especially because they may be an early signal of systemic disease.

Causes of erectile dysfunction

Organic causes

  • vascular disease — atherosclerosis, hypertension, high cholesterol,
  • diabetes and metabolic disorders,
  • testosterone deficiency and other hormonal disorders,
  • prostate disease or status after urological procedures,
  • neurological diseases such as multiple sclerosis or Parkinson’s disease,
  • medications: some antihypertensive drugs, antidepressants, prostate medications.

Psychological and mixed causes

  • stress, anxiety, depression,
  • performance anxiety, a vicious cycle where fear of failure worsens the problem,
  • relationship problems,
  • chronic fatigue and burnout.

Lifestyle

  • smoking — one of the stronger risk factors,
  • obesity and lack of physical activity,
  • excessive alcohol consumption, lack of sleep.

Erectile dysfunction and the heart. Vascular erection problems may precede symptoms of cardiovascular disease by several years. A urologist pays attention to this during the consultation and, if needed, refers the patient to the appropriate specialist.

When should you see a doctor?

  • the problem recurs for several weeks,
  • you notice gradual worsening of erections over several months,
  • you have diabetes, hypertension or cardiovascular disease,
  • erection problems are accompanied by decreased libido or other hormonal symptoms,
  • the problem affects your relationship or well-being,
  • you continue to struggle despite lifestyle changes.

How do we diagnose and treat erectile dysfunction?

We start with a conversation — detailed, unhurried and without judgment. The doctor takes a medical history, assesses risk factors and matches diagnostics and treatment to the specific case.

Frequently asked questions about erectile dysfunction

Can erectile dysfunction be treated?

In many cases, significant improvement or complete resolution of symptoms is possible — it depends on the cause. Psychogenic erectile dysfunction often responds well to psychotherapy. Vascular and hormonal erectile dysfunction requires targeted treatment. The urologist assesses the prognosis individually after diagnostics.

Can erectile dysfunction be a symptom of a serious disease?

Yes. Vascular erectile dysfunction may be an early signal of coronary artery disease, hypertension or diabetes — sometimes preceding other symptoms by several years. That is why it is worth examining the problem rather than waiting.

What does a urologist do during the first visit?

The doctor takes a detailed history: duration of the problem, chronic diseases, medications and lifestyle. If needed, the doctor orders tests such as testosterone level, lipid profile, blood glucose and penile Doppler ultrasound. The first visit is a conversation, not a procedure.

What is the difference between a urology and an andrology consultation?

A urology consultation covers broad urinary and genital health problems. An andrology consultation focuses on male sexual health — erections, libido, fertility and hormones. In erectile dysfunction, we often start with a urology or andrology consultation — the doctor will suggest which pathway is appropriate.

Are erection medications, such as PDE5 inhibitors, enough?

PDE5 inhibitors such as sildenafil and tadalafil are an effective class of medications for many men — but they have contraindications, interact with heart medications and do not treat the underlying cause. The urologist will assess whether they are the right choice and at what dose.

What are B-Shot and P-Shot in erectile dysfunction treatment?

B-Shot with botulinum toxin and P-Shot with platelet-rich plasma are aesthetic urology procedures considered in selected patients after a full urology consultation. They are not suitable for everyone — the choice depends on the individual case and the doctor’s assessment.

How soon can I get an appointment with a urologist in Gdańsk?

At Wyspa Medycyny Przyjaznej, appointments are usually available within a few business days. Book through ZnanyLekarz or call: 600 006 913.
