Skin rejuvenation should begin by defining exactly what needs improvement. If the problem is dynamic wrinkles, botulinum toxin or baby botox is often considered. Which procedures can be considered for skin rejuvenation?

Below we describe possible treatment directions, not a final qualification. The final choice of method should be made after consultation with Dr. Piotr Rak.

1. Botulinum toxin

Most often considered for dynamic wrinkles: the forehead, frown lines, crow’s feet and selected facial areas. The aim is to reduce excessive muscle activity, not to “freeze” facial expression.

2. Baby botox / mesobotox

It may be discussed with patients who expect very subtle refreshment, smoothing of fine lines and a natural effect. The scope depends on facial expression, skin quality and expectations.

3. Hyaluronic acid

Used for volume loss, folds, wrinkles, lip shaping, facial volumetry and improving hydration in selected areas. It should not be used to “overdraw” the face, but to restore proportions.

4. Facial volumetry

For patients with volume loss in the cheeks, temples or lower face. It may improve proportions and reduce a tired appearance if the problem is tissue hollowing.

5. Poly-L-lactic acid PLLA

A biostimulating method aimed at gradually improving firmness, volume and skin quality by stimulating collagen remodelling. The effect develops in stages, so it requires patience and proper qualification.

6. Lifting threads

Considered in cases of mild or moderate laxity, tissue sagging and the need to improve facial contour without classic surgery. They provide mechanical lifting and may support tissue remodelling.

7. CO2 laser

A direction for patients with uneven skin texture, fine wrinkles, scars, enlarged pores, pigmentation or the need to improve skin quality on the face, neck and décolleté.

8. CO2 laser scar reduction

If the main problem is acne scars, uneven skin surface or post-inflammatory structural changes, the appropriate pathway may be laser scar treatment rather than classic fillers.

9. Facial mesotherapy

10. Lipotransfer

Autologous fat transfer may be considered for volume loss and the need for natural filling of selected areas. This is a more procedural pathway than classic “skin care”.

11. Lip hydration and shaping

In some patients, facial rejuvenation also involves the lips: loss of hydration, fine lines around the mouth, loss of vermilion volume or upper lip proportions. In such cases, HA or other solutions are discussed.

12. Aesthetic-surgical procedures as a separate pathway

If the problem is drooping eyelids, excess skin or lip proportions, the appropriate pathway may be eyelid surgery or lip lift. These are separate procedures, not “general skin rejuvenation”.

How to match the method to the problem?

Good qualification means matching the mechanism of the procedure to the skin problem. Not every “rejuvenating” procedure works in the same way.

  • Dynamic wrinkles — more often botulinum toxin, baby botox or combined treatment.
  • Volume loss — hyaluronic acid, volumetry, lipotransfer or poly-L-lactic acid.
  • Laxity and tissue sagging — lifting threads, PLLA, CO2 laser or a staged plan.
  • Skin quality and hydration — mesotherapy, HA, biostimulation, CO2 laser.
  • Scars and pores — CO2 laser, skin texture treatment, sometimes combined treatment.
  • Mouth area — hyaluronic acid, lip hydration, shaping or, in selected cases, lip lift.
  • Drooping eyelids — not always aesthetic medicine; sometimes consultation for plastic eyelid correction is appropriate.

When is one procedure not enough?

In many patients, skin ageing is multifactorial. Dynamic wrinkles, volume loss, changes in the facial oval, poorer skin quality and photoageing may appear simultaneously. In such cases, Dr. Piotr Rak may propose a staged plan, for example first improving skin quality, then volumetry, and finally a gentle correction of facial expression.

Combined treatment does not mean performing many procedures at once. Sometimes it is safer and more natural to divide therapy into several visits, assessing the result after each stage.

When might a procedure not be the best first step?

  • when the skin has an active infection, herpes, inflammation or fresh irritation,
  • when the patient has unrealistic expectations about the result,
  • when the problem first requires dermatological diagnosis,
  • when there are uncontrolled chronic diseases, coagulation disorders or contraindications to a specific product,
  • when the patient does not accept the risk of swelling, bruising, asymmetry, pigmentation, lumps or a healing period,
  • when skin care, photoprotection, acne treatment, redness treatment or lifestyle improvement may be sufficient,
  • when the problem is more surgical than aesthetic-procedural, e.g. significant excess eyelid skin.

How to prepare for the consultation?

  • consider whether wrinkles, laxity, volume, facial oval, scars, pores, pigmentation or lips bother you the most,
  • prepare information about previous aesthetic medicine procedures and products used,
  • mention chronic diseases, allergies, anticoagulant medication, tendency to keloids and herpes,
  • do not plan a procedure just before an important event if you do not know how you react to swelling or bruising,
  • bring photos from several years ago if you want to show a natural point of reference,
  • tell the doctor whether you want a quick effect, a natural staged change or gradual improvement in skin quality,
  • do not hide previous fillers, complications or procedures performed outside a medical facility.
