Improving the appearance of the penis first requires defining exactly what the patient wants to change. A problem with visible length requires a different method, girth another one, the glans another, and the foreskin, frenulum, scar, pearly penile papules or Fordyce spots yet another. What can “improving the appearance of the penis” mean?

Men come in with different needs. Some patients want to increase penile girth. Others want to improve visible length because the penis seems too short or partially hidden. This is precisely why it is not worth starting with choosing a procedure from the internet. The same symptom — “I do not like the appearance of my penis” — may lead to completely different solutions. Sometimes education and explanation of anatomy will be best; sometimes treatment of phimosis; sometimes girth enlargement; sometimes suspensory ligament release; and sometimes laser removal of superficial lesions.

Consultation — the most important stage

During the consultation, the doctor determines whether the problem concerns aesthetics, function, skin, erection, length, girth, glans or foreskin. It is also important to discuss expectations. Aesthetic urology may improve selected aspects of appearance, but it should not be presented as a guarantee of an ideal result or a large change in every patient.

The doctor may perform an examination and assess anatomy, proportions, skin, foreskin, frenulum, glans, scars and any superficial lesions. If the problem is a short penis, it may be important to distinguish anatomical length from visible length. If the problem is girth, the doctor discusses volumetric methods. If the problem is papules or spots, their benign nature must first be confirmed.

Safety principle: we do not choose a procedure before diagnosing the problem. First we define the goal: length, girth, exposure, glans, skin, scar, foreskin, frenulum or superficial lesions.

Possible pathways for improving penile appearance

1. Visible penile length

If the patient feels that the penis is too short, the doctor assesses whether the issue concerns actual length, a hidden penis, suprapubic fat tissue, scars or subjective dissatisfaction. In selected patients, suspensory ligament release may be discussed.

2. Shaft girth and volume

When the problem is insufficient girth or unfavourable proportions, volumetric methods may be considered, such as hyaluronic acid, poly-L-lactic acid or autologous fat. These methods are not simple “lengthening”; they usually concern volume and proportions.

3. Poly-L-lactic acid

Poly-L-lactic acid may be discussed as a biostimulating method aimed at gradual improvement of tissue volume and quality. It requires qualification, patience and understanding that the effect develops differently than after a classic filler.

4. Autologous fat

The patient’s own fat may be used in selected cases to improve volume. The result depends on harvesting, preparation, healing and individual graft take.

5. Glans and proportions

If the patient is bothered by the appearance of the glans, its proportion to the shaft or subjectively small volume, the doctor may discuss glans enlargement, for example with hyaluronic acid or regenerative methods, if justified.

6. Fibrin / PRF and revitalisation

Platelet-rich fibrin, PRF or revitalisation procedures may be discussed as support for tissue quality, regeneration or function. They should not be presented as a simple method of penile lengthening.

7. Pearly penile papules and Fordyce spots

Small papules or spots on the penis may be benign lesions, but they may also resemble infections or other skin diseases. Assessment is needed first. If the doctor confirms their benign nature, laser removal may be discussed.

8. Foreskin, phimosis and circumcision

The appearance of the penis may be affected by phimosis, scarred foreskin, an uneven foreskin edge, recurrent inflammation or the result of a previous procedure. In such cases, the solution may involve phimosis treatment, foreskin plasty or circumcision.

9. Frenulum and skin tension

A short frenulum may cause pain, tearing, pulling of the glans and an unfavourable appearance during erection. In such cases, frenuloplasty or frenulum release should be discussed.

When can improving penile appearance make sense?

A consultation may be justified when the appearance of the penis causes persistent discomfort, interferes with intimate relationships, affects self-confidence or is associated with a real anatomical problem. A reason to speak with a doctor may also be a condition after a previous procedure, a scar, phimosis, asymmetry, papules, spots, a feeling of insufficient girth or a problem with visible length.

However, the doctor should distinguish a situation in which a procedure may genuinely help from one in which the main problem is unrealistic expectations, comparison with internet materials or strong anxiety about appearance despite normal anatomy.

When might a procedure not be the best first step?

  • when the problem mainly concerns erectile dysfunction rather than appearance,
  • when the patient expects guaranteed, significant lengthening during erection,
  • when there is an active infection, inflammation, ulceration or unexplained skin lesion,
  • when expectations are unrealistic or change from visit to visit,
  • when the patient does not accept possible complications: swelling, irregularities, scar, discoloration, pain or the need for correction,
  • when diagnosis of Peyronie’s disease, phimosis, short frenulum or hidden penis is needed first,
  • when uncontrolled chronic diseases or contraindications to the procedure are present.

What should you avoid?

  • do not inject any preparations into the penis yourself,
  • do not use fillers or “oils” from uncertain sources,
  • do not remove papules, spots or skin lesions using home methods,
  • do not use wart preparations in the penile area without a diagnosis,
  • do not choose a procedure based only on “before and after” photos,
  • do not skip diagnostics for pain, curvature, scars, phimosis, skin tearing or erectile dysfunction,
  • do not expect one method to solve every aesthetic and functional problem.

How should you prepare for the consultation?

  • define exactly what you want to improve: length, girth, glans, skin, scar, papules, foreskin, frenulum or proportions,
  • say whether the problem concerns the flaccid state, erection or both situations,
  • inform the doctor about previous procedures, circumcision, fillers, injuries or infections,
  • mention erectile dysfunction, pain, curvature, skin tearing or difficulties with the foreskin,
  • prepare a list of medications, chronic diseases, allergies and blood clotting disorders,
  • consider what result you would regard as a real improvement,
  • do not hide previous injections or procedures performed outside a medical facility.
