Double chin

aesthetic medicine · double chin · liposuction · thread lifting · Gdańsk

A double chin, also called submental fullness, may have different causes. In one patient, local excess fat under the chin is the main issue. In another, the problem may be loose skin, weaker jawline definition, chin anatomy, tissue laxity in the upper neck or reduced skin quality. That is why treatment should begin with a consultation with Dr Piotr Rak, followed by a personalised choice of method: chin liposuction, thread lifting or, in selected cases, mesotherapy.

Key answer

A double chin should be treated according to its cause. If local submental fat is the main issue, chin liposuction may be considered. If loose skin and tissue laxity dominate, thread lifting may be a more appropriate direction. If the skin is thin, dehydrated, less firm or needs quality improvement, mesotherapy may be used as a supportive option. Some patients may benefit from a staged combined plan, but not every patient qualifies for every method.

What is a double chin?

A double chin is visible fullness or excess tissue under the chin and in the upper neck. It may form a fold, a soft bulge, rounded fullness under the jaw or loss of a clear transition between the face and the neck. For many patients, this is an aesthetic concern because the face may appear heavier, less defined or older.

In practice, a double chin may have several mechanisms. It may be caused by local fat accumulation, skin laxity, weak chin projection, tissue descent, neck anatomy, previous weight changes, skin ageing or family predisposition.

How do patients describe this problem?

  • “I have a double chin”,
  • “a fold has appeared under my chin”,
  • “my jawline is not visible in profile photos”,
  • “I have fat under my chin despite a normal body weight”,
  • “my neck looks shorter and heavier”,
  • “the skin under my chin is loose”,
  • “after losing weight, loose skin remained”,
  • “my jawline has become less defined”,
  • “my chin and neck area makes my face look older”.

Common causes of a double chin

1. Excess submental fat

In some patients, the main issue is a local fat deposit under the chin. It may persist despite diet and physical activity, especially when it is related to genetics or facial anatomy. In such cases, chin liposuction may be discussed after medical qualification.

2. Skin laxity

If the skin is thin, loose and less elastic, removing fat alone may not provide an ideal result. When laxity dominates, the doctor may consider methods that improve tissue support and tension, including thread lifting or treatments aimed at skin quality.

3. Loss of jawline definition

A double chin may be more visible when the jawline is less defined, the chin has weaker projection or tissues in the lower face descend. Therefore, the entire facial profile should be assessed, not only the fold under the chin.

4. Changes after weight loss

After significant weight loss, fat may decrease, but the skin may not contract sufficiently. The result may be loose skin under the chin or on the neck. In such cases, liposuction is not always the best answer; skin tightening and quality improvement may be more important.

5. Anatomy and genetics

Some people have a visible double chin despite a slim figure. This may be caused by chin and neck anatomy, jawline structure, fat distribution and skin characteristics. A consultation should include assessment of the face from the front, profile and during movement.

Important: chin liposuction is not a treatment for obesity and is not a general facial slimming method. It is a contouring procedure for a selected area in patients who qualify for the treatment.

When is it worth booking a consultation?

  • when a double chin is visible despite stable body weight,
  • when local fat under the chin appears to be the main issue,
  • when the skin under the chin has become loose and lax,
  • when the jawline is less defined,
  • when the problem became more visible after weight loss,
  • when the patient does not know whether liposuction, threads or skin-quality treatment is appropriate,
  • when a double chin affects the facial profile in photos,
  • when the goal is a more defined chin and neck contour without an exaggerated effect.

Which methods may be considered?

The following are treatment directions, not automatic recommendations. Qualification is made by Dr Piotr Rak after examining the skin, fat tissue, jawline, neck and patient expectations.

1. Chin liposuction

Chin liposuction may be considered when the main issue is local fat under the chin. The goal is to reduce fullness in this area and improve the transition between the face and neck.

It is not suitable for everyone. If the skin is very loose or the problem is mainly related to chin and jawline anatomy, liposuction alone may be insufficient.

2. Thread lifting

Thread lifting may be considered when loose skin, tissue descent and loss of contour dominate rather than fat alone. Threads may provide support and improve tension in the lower face and submental area.

When there is a significant excess of skin, the effect of threads may be limited. In such cases, the doctor should clearly discuss realistic possibilities of minimally invasive treatments.

3. Mesotherapy

Mesotherapy may be a supportive option when the issue is skin quality: dryness, thin skin, reduced firmness or the need to improve skin condition. It should not be treated as a replacement for liposuction when there is clear excess fat, or as a substitute for lifting in marked skin laxity.

It may be part of a supportive plan if the doctor decides that improving skin quality is relevant to the overall result.

4. Combined treatment

Some patients may benefit from a staged plan, for example chin liposuction when fat is dominant, followed by work on skin tension or contour using threads or mesotherapy. The sequence depends on the examination.

How is the method matched to the cause?

The choice depends on what dominates during the examination. If local, soft fat is present under the chin and the skin has good elasticity, liposuction may be discussed. If fat is not the main problem and loose skin or tissue descent is more visible, thread lifting may be more appropriate. If the issue is subtle and related to skin quality, mesotherapy may be considered as supportive treatment.

It is a mistake to treat every double chin in the same way. Removing fat when the skin is lax may make looseness more visible. Threads alone may not provide a satisfying result if fat volume dominates. Mesotherapy may improve skin condition, but it does not replace surgical contouring when there is clear fat excess.

When may treatment not be the best first step?

  • when body weight is changing rapidly and is not stable,
  • when the patient expects a major neck-lift effect from a minimally invasive procedure,
  • when the issue is mainly related to chin or jawline anatomy rather than fat tissue,
  • when the skin is very loose and has poor elasticity,
  • when there is an active infection, skin inflammation or fresh irritation,
  • when the patient takes anticoagulants or has clotting disorders without prior medical assessment,
  • when expectations are unrealistic or the patient does not accept the risk of swelling, bruising, irregularity, asymmetry, scarring or possible correction,
  • when there is an unexplained lump, enlarged lymph node or lesion under the chin requiring diagnostic assessment.

What not to do?

  • do not assume that every double chin is caused by fat,
  • do not choose liposuction only because the chin area looks fuller — skin, fat and contour must be assessed,
  • do not expect mesotherapy to remove a clear excess of fat,
  • do not expect threads to replace a surgical neck lift when skin laxity is significant,
  • do not undergo procedures with people who do not have appropriate medical qualifications,
  • do not hide previous procedures, scars, medical conditions, anticoagulant medication or healing problems.

How to prepare for consultation?

  • consider whether the problem is mainly fat, loose skin, jawline contour or facial profile,
  • prepare information about recent weight changes,
  • tell the doctor about previous treatments of the face, neck, chin and submental area,
  • inform the doctor about anticoagulants, chronic diseases, allergies, tendency to scarring and wound-healing problems,
  • do not plan treatment immediately before an important event if you do not know how you react to swelling or bruising,
  • bring profile photos from a few years ago if you want to show your natural contour,
  • tell the doctor whether you expect a clear volume reduction, skin tightening or subtle skin-quality improvement.

FAQ – double chin

Does a double chin always mean excess fat?

No. A double chin may result from local excess fat, but also from skin laxity, chin anatomy, weaker jawline definition, tissue descent or changes after weight loss.

When is chin liposuction considered?

Chin liposuction may be considered when the main issue is local fat under the chin and the patient qualifies for treatment after medical examination.

When may thread lifting be better?

Thread lifting may be considered when loose skin, tissue descent and loss of contour dominate, rather than fat alone. It does not replace a surgical neck lift in marked skin excess.

Does mesotherapy remove a double chin?

Mesotherapy may support skin quality and condition, but it should not be treated as a replacement for liposuction when there is clear fat excess, or as a replacement for lifting when skin laxity is significant.

Can chin liposuction and threads be combined?

In selected patients, combined or staged treatment may be considered, but the sequence and scope should be determined by the doctor after assessing skin, fat tissue and jawline contour.

Does chin liposuction slim the whole face?

Chin liposuction is a contouring procedure for a selected area. It is not a treatment for obesity or general facial slimming. Qualification is made by the doctor.
