foreskin-preserving surgery — the foreskin stays
Preputioplasty is a surgical procedure that widens a narrowed foreskin without removing it. It is an alternative to circumcision for patients whose narrowing can be corrected while preserving the foreskin.
Whether preputioplasty is possible in your case is decided by the urologist after examination — not every narrowing qualifies for the foreskin-preserving method.
Qualification takes place during a urological consultation. Appointments are usually available within a few days.

Preputioplasty is one of the methods of treating phimosis in adults — a narrowing of the foreskin that makes it difficult to retract over the glans. Unlike circumcision, the procedure does not remove the foreskin; it widens the constricting ring.
If you are wondering whether your phimosis requires surgery at all, start with the article Phimosis — when to operate? This page already describes a specific procedure and is intended for those considering the foreskin-preserving method.
The procedure involves incising the constricting ring of the foreskin and closing the wound in such a way that the foreskin becomes wider and can be retracted freely. The surgeon incises the foreskin along the axis of the narrowing and then closes the incision transversely — which widens the circumference at the point that previously formed the obstruction.
Depending on the anatomy and extent of the narrowing, the doctor may use a single incision or several incisions distributed around the circumference. For narrowings with scarring, a Z-plasty technique is sometimes used to distribute tissue tension. The choice of technique is made intraoperatively, after assessing the tissues.
The procedure is performed on an outpatient basis, under local anaesthesia. It usually takes 30–45 minutes. The patient goes home the same day. Sutures are most often absorbable and do not require removal.
The procedure is considered for adult men who:
Qualification is carried out by a urologist during a urological consultation — assessing the degree and nature of the narrowing and the condition of the tissues.
Not every narrowing can be corrected with a foreskin-preserving method. Honest qualification matters more here than the mere technical possibility of performing the procedure — preputioplasty carried out against the indications may not produce a lasting result.
Situations in which the doctor may recommend circumcision instead of preputioplasty:
In such cases the doctor will discuss circumcision as a solution giving a more durable result. The decision is made together with the patient, after explaining the differences between the methods.
| Preputioplasty | Circumcision | |
|---|---|---|
| Foreskin | preserved | removed partially or completely |
| Reversibility | tissue remains — later circumcision still possible | irreversible |
| Risk of recurrent narrowing | present | practically absent |
| With extensive scarring | usually not indicated | method of choice |
| Procedure page | this page | circumcision → |
Both methods are recognised ways of treating phimosis. The choice depends on the condition of the tissues, the patient's expectations and medical assessment — not on the price of the procedure.
The full list of contraindications is discussed individually by the doctor during qualification.
Healing usually takes about two weeks. In the first days after the procedure, swelling and tenderness of the operated area are typical — they subside gradually.
Fever, increasing pain not responding to painkillers, swelling that grows after 48 hours, discharge from the wound or suture dehiscence — these are symptoms requiring contact with the clinic. Mild discomfort in the first days is typical.
The urologist assesses the degree of narrowing and the condition of the tissues, and answers whether preputioplasty is possible in your case or whether circumcision would be the better solution. Risks and realistic outcomes are discussed.
Local anaesthesia, incision and widening of the constricting ring, closure of the wound. About 30–45 minutes. The patient goes home the same day.
About two weeks. Instructions on hygiene, exertion and intercourse are given to the patient in writing after the procedure.
A follow-up visit allows healing and the achieved foreskin mobility to be assessed. The timing is set by the doctor individually.

The urologist will assess the narrowing and tell you plainly whether the foreskin-preserving method will give a lasting result.
No. Preputioplasty widens the foreskin while keeping it intact. Circumcision involves partial or complete removal of the foreskin. These are two different procedures with different indications — the comparison is in the table above.
Yes — unlike circumcision, there is a risk of recurrent narrowing because the foreskin tissue remains. The risk is lower with correct qualification and adherence to aftercare instructions. If the narrowing recurs, the doctor will discuss further options — including circumcision.
The procedure is performed by the clinic's urologists: Jakub Kłącz, MD, PhD, Jakub Krukowski, MD, PhD, Jakub Gondek, MD and Arthur Abbazov, MD. Availability is shown in the clinic calendar → If you have no preference, our reception will help you choose a doctor and an appointment time.
The procedure is performed under local anaesthesia, which limits sensation during it. Once the anaesthetic wears off, discomfort and swelling may appear — usually relieved by over-the-counter painkillers. Individual sensations may vary.
The procedure preserves the foreskin, so it does not remove the tissues responsible for sensation. Sensation at the site of the surgical scar may change temporarily — it usually returns to normal during healing. Any concerns are worth discussing with the doctor beforehand.
Usually after about 2–4 weeks, once the wound has healed — the exact timing is determined by the doctor at the follow-up. Resuming too early increases the risk of suture dehiscence and prolongs healing.
In some cases a short frenulum coexists with a narrowed foreskin — both problems can then be addressed during a single procedure. The extent of the intervention is assessed by the doctor during qualification. More on frenuloplasty.
The procedure costs 3000 PLN. The qualifying consultation is billed separately — current rates are in the urology price list. The final scope of the procedure and its price are confirmed by the doctor after qualification.
